Using this Page

This page is your central hub for creating and using the Checkpoints First-Year Driving Agreement.

From here you can:

• Create a new agreement for your teen by clicking on “Create New Agreement”, entering a name, and clicking “Create”. You may create more than 1 agreement by returning to this page and repeating the above instructions.
• View, change and/or print a saved agreement by clicking on the name for the agreement.
• View and/or print a sample agreement that has already been completed by clicking on the “Sample Agreement” button above.
• After creating your first agreement,  you will see a button to take the evaluation survey. This is required if you would like a printable certificate of completion.
• You can return to this page to modify your agreement at any time, your agreement will be saved.
